Is digital remote monitoring suitable for long-term conditions? Clinical Pharmacy Congress North highlights
Marsha Showman and Reena Patel (Leeds Health and Care Partnership) presented a poster describing how utilisation of health services was improved when people with long term conditions, including COPD, heart failure and type 2 diabetes, took part in a remote monitoring project.
Patients were provided with suitable devices and an app that allowed secure communication with their GP practice. Another benefit of the scheme was that it gave patients more confidence and reassurance in managing their long-term conditions at home.
